Abstract
The use of electronic health records to support health activities is currently quite widespread, due to the increasing complexity of health care and the need to optimize human and material resources. The requirement of health care professionals to access clinical information expeditiously is a key factor in the quality of health care services. Therefore, the assessment of the usability of the health of applications is crucial. The objective of this study was to evaluate the usability of SClinico through a questionnaire especially developed, and it took place in an hospital environment. The SClinico is an electronic health record application used on a large scale in public Portuguese healthcare institutions. The results show that SClinico has some usability issues that need to be improved. Among these we highlight the need to take a holistic view of the patient's health record and simultaneously quick access to relevant details for different clinical situations.

The aging of population worldwide leads to the emergence of new needs with regard to the provision of care, including continuity and integration of care. Obtaining a holistic view of the users, and have access to this information on the place and time it is needed, is a key aspect to achieve an improvement in care activities. In this paper we propose a Platform of Services to support and build complex applications for the registration of information, which should enable the integration of health care and social assistance. Abstract
The population ageing requires new approaches to optimize the available resources. In particular, it is urgent to evolve for an effective care integration, which requires toconsider aspects of people's daily lives that impact on their health conditions. Therefore, the information interoperability has an increasing importance, not only the interoperability of health related information, but also social context related information. In this paper, we propose a framework for the creation of applications for information recording, based on a generic and flexible information model, for the provision of integrated care.

Application for VTE Stratification and Risk Assessment

Abstract
Venous thromboembolism is a pathology that is responsible for high rates of morbidity and mortality in hospitals. An adequate thromboprophylaxis may contribute to an effective improvement of the well-being and safety of patients. One of the important factors for the prescription of a thromboprophylaxis is the determination of the risk of thromboembolism, which can be implemented in several ways, using different methodologies, namely by using risk scales. In this work, developed in the Hospital Center of Tras-os-Montes and Alto Douro, Portugal, we describe the development and implementation of an application with two scales for the assessment and stratification of the risk of thromboembolism, to be applied in hospitalized patients (Caprini scale) and in patients in ambulatory surgery (scale recommended by the Portuguese Ambulatory Surgery Association). The application was integrated into the hospital electronic health record system (SClinico).
