Integrated care of the elderly and the continuous development and adaptation of information systems

Abstract
There is a need to overcome problems resulting from disconnected and fragmented healthcare and social assistance services when it comes to provide integrated care services for the elderly. The concept of integrated care services requires healthcare information services, such as Electronic Health Record, to be up and running not only in hospitals and other healthcare units, but also in social assistance institutions since they take on an important role in providing care for the elderly. This paper proposes a reference model that identifies the processes, procedures and concepts inherent to the functioning of social assistance institutions. This model coupled with an implementation architecture whose flexibility and modularity allows for the gradual development of applications and components reuse. This makes possible to introduce and adapt information services through the involvement of the care providers in a sector that traditionally has had difficulties to incorporate Information and Communication Technology. © 2010 IEEE.

Abstract

Automated endoscopic capsule analysis using a Grid computing environment

Abstract
With the increase of Endoscopic Capsule (EC) exams, the amount of video data to be analyzed by automated image processing algorithms grows exponentially and standard desktop computers start to present limitations to complete that procedure in an acceptable "clinical time". This has motivated us to assess the potential of Grid computing paradigm to process and analyze EC data, using existing Grid infrastructures. We used our Automated Topografic Segmentation (ATS) algorithm as a case study to run experiments on the Ibergrid Grid infrastructure. As a result, we were able to port the existing ATS algorithm used in routine to run on the Grid seamlessly. A friendly portal environment was adopted, allowing for clinical end-users to harness from existing production Grids in a practical way. The processing time could be reduced in some cases but, when considering job processing overheads, the desktop version is preferable for small scale processing. These results show that Grid computing is a promising technology to process massive amounts of EC data, but current infrastructures present a high variability in jobs processing times, especially when compared with dedicated clusters.

Preliminary Analysis of the Psychometric Properties of Ryff's Scales of Psychological Well-Being in Portuguese Adolescents

Abstract
This article presents a set of research studies that aim to adapt Carol Ryff's scales of psychological wellbeing (SPWB) and to analyze its psychometric properties in adolescents. The first two studies focused on the reliability and factorial validity of different Portuguese short versions of SPWB, revealing measurement models inadequacies and low internal consistency. In the third study we developed a shortened version (30 items), taking into account the application of psychometric criteria suggested by van Dierendonck (2005). The scales of this version revealed better reliability and adequate goodness of fit indices for the six-factor model, as proposed by Carol Ryff's PWB theory. Although further research focused on the psychometrical properties reanalysis of this shortened version of SPWB is needed, this article provides a contribution to the research and intervention on positive mental health during adolescence.

Confirmatory Factorial Analysis of TEOSQp

Abstract
The present research assessed the reliability, factorial validity and measurement invariance (by gender) of the Portuguese version of TEOSQ - Task and Ego Orientation in Sport Questionnaire (Fonseca & Biddle, 2001). Data were collected from 1010 physical education students with a mean age of 15.42 (SD=1.91). Factorial and invariance measurements were tested via confirmatory factorial analysis. Results supported internal consistency for the two proposed subscales (task and ego). Based on modification indices and theoretical justification the TEOSQp was reduced to 12 items with better goodness-of-fit indices for the oblique model. The results of gender invariance did not provide full empirical support to the multi-group equivalence assumption, being suggested that TEOSQp does not measure in the same way goal orientations for boys (orthogonal model) and girls (oblique model). In light of these results, conceptual, empirical and practical issues were discussed.

Confirmatory Factorial Analysis of the Brazilian Version of the Competitive State Anxiety Inventory-2 (CSAI-2)

Abstract
The aim of this study was to assess the factorial structure of the Brazilian version of CSAI-2, using several structures suggested in previous studies. Two samples consisted of Brazilian soccer players, 266 from regional level (age 22.87 +/- 4.08 years; athletic experience 11.32 +/- 4.15 years) and 263 from national level (age 23.16 +/- 4.37 years; athletic experience 11.11 +/- 4.78 years) were used. The CSAI-2 is a 27-item inventory that measures negativism, physiological activation and self-confidence in a competitive setting. The results of CFA according to the original structure showed some inadequacy of the model. The model proposed by Cox, Martens, Russell (2003), composed of three factors (17-item), demonstrated better adjustment to the regional level sample (chi 2/df = 1.871, CFI = .934, GFI = .916, RMSEA = .057), while the model suggested by Coelho, Vasconcelos-Raposo, Fernandes (2007), composed of two factors (18-item), adapted better to the national level sample (chi 2/df = 1.701, CFI = .924, GFI = .914, RMSEA = .052). When we analyzed the two samples together, Coelho et al. (2007) was the better model, because it displayed greater invariance. The use of this model was suggested in the assessment of intensity of negative thoughts and the subsequent confirmation of its psychometric properties is recommended.
